A continuum of care is a coordinated approach that ensures patients receive the right level of care at the right time without unnecessary gaps or disruptions. Instead of navigating multiple providers on their own, patients benefit from a seamless experience that keeps everyone focused on one goal – improving health and quality of life.




Every patient has unique needs that can change over time. Some may begin with skilled nursing and therapy after a hospital stay. As they regain independence, they may no longer require skilled medical services but still need assistance with daily activities such as bathing, meal preparation, medication reminders, or transportation.
